Lesson 8 Β· X-rays, sound-pictures & echoes

Seeing Inside

A NICU baby can't say what hurts. So doctors need to look inside β€” at lungs, brain, heart, and belly β€” without opening anything. They do it with two kinds of invisible rays: super-light and super-sound.

🩻 The X-ray: a shadow picture

X-rays are a kind of light too energetic for eyes to see β€” so energetic it flies through soft parts of the body. Dense things like bone block it (showing up white on the picture), squishy organs slow it a little (gray), and air lets it sail through (black). So an X-ray is really a shadow picture of what's dense inside you.

In the NICU, a tiny X-ray plate slides under the baby, a machine hums for a fraction of a second, and the team gets answers: How do the lungs look β€” airy black, or hazy from RDS? Is the breathing tube's tip sitting in exactly the right spot? Did the PICC line reach the right place? Is there worrying gas in the wall of the gut (a telltale sign of NEC)? X-rays use a tiny bit of radiation, so doctors use them like hot sauce: whenever needed, never carelessly.

πŸ”Š The ultrasound: seeing with sound

Shout at a canyon wall and the sound bounces back β€” an echo. Bats and dolphins use echoes to β€œsee” in the dark: send a sound, listen for the bounce, figure out what's out there. An ultrasound machine does exactly this, with sound pitched far too high for ears to hear. The wand presses gently on the skin (with warm goo, to help sound cross), fires millions of tiny sound pips into the body, times their echoes off the organs, and paints a live moving picture from them. No radiation at all β€” you can scan a baby every day, right at the bedside, totally safely. It's the NICU's favorite camera.

But there's a problem with looking at the brain: sound bounces off bone, and the brain lives in a bone helmet. Here's the elegant trick β€” babies have a built-in window. The skull is born in separate plates that haven't knitted together yet (so the head can squeeze through during birth), leaving a soft, bone-free diamond on top: the fontanelle, or β€œsoft spot.” Rest the ultrasound wand right there and sound passes cleanly through β€” a perfect porthole into the brain. Nature left a door, and doctors found it.

πŸ’” Real talk: brain bleeds (IVH)

Remember Lesson 4's fragile brain vessels? In babies born very early, those vessels can rupture and bleed into the fluid spaces (ventricles) of the brain β€” an intraventricular hemorrhage (IVH). It usually happens in the first days of life, often silently. That's why every baby born before about 30–32 weeks gets a screening head ultrasound through the fontanelle in the first week, whether or not anything seems wrong.

Doctors grade what they find from 1 to 4. Grades 1–2 (small bleeds) are common and usually heal β€” most of these babies do well. Grades 3–4 (the ventricle filling with blood, or bleeding into the brain tissue itself) are serious: they can block the brain's fluid drainage (sometimes needing surgery to relieve the pressure) and can cause lasting disabilities like cerebral palsy β€” and the worst bleeds can be fatal. There's no undo button for a bad bleed, which is why so much NICU care β€” gentle handling, steady blood pressure, steady oxygen β€” is really about preventing this. The scan through the soft spot is how the team knows the truth, faces it with the family, and plans what comes next. Honest looking is a kindness.

πŸ«€ The echo: an ultrasound of the heart

Point an ultrasound at the heart and you get an echocardiogram (β€œecho”) β€” a live movie of the chambers squeezing and the valves flapping, with a color mode that paints blood flow red and blue on screen so doctors can watch which way blood is moving. In the NICU, the echo's number one customer is a leftover from life in the womb…

The PDA story. Before birth, a baby's lungs are folded up and full of fluid β€” the placenta does the oxygen job. So the unborn heart uses a clever detour: a short vessel called the ductus arteriosus that lets blood skip the not-yet-working lungs. After birth, with the first breaths, the lungs take over β€” and the detour is supposed to seal itself shut within a few days. In full-term babies it usually does. But in preemies it often stays stuck open β€” that's a patent ductus arteriosus (PDA) (β€œpatent” is doctor-speak for β€œopen”), and it's very common in the smallest babies. An open detour lets blood flood the lungs and steal flow from the body. The echo sees it, measures it, and helps the team choose: wait patiently (many close on their own), give a medicine that nudges the vessel to tighten (like ibuprofen β€” yes, the headache medicine), or, for stubborn cases in even the tiniest babies, thread a miniature plug through a leg vein and close the detour without any open surgery at all.

An unborn baby's whole circulation is different: it has THREE built-in shortcuts that all must close after birth, on cue, without anyone touching anything. Blood flow literally reorganizes itself in the first minutes and days of life β€” you did this yourself once, and nailed it. Doctors call the change transitional circulation, and half of NICU heart problems are just this switchover happening late or wrong.

Then try echolocation: stand a few meters from a big wall, clap once, and listen for the slap-back echo.
